I don't think there's a single file for it. Fab's eyepatch pack contains 7 DDS files, which covers every instance of your face in the game, as far as I know.

You're after:
(Pak4) <b>player_head_yng.dds</b>
(Pak7) <b>player_head_yng_rags.dds</b>
(Pak4) <b>player_head_yng_white_eyes.dds</b>
(Pak6) <b>player_head_mid.dds</b>
(Pak3) <b>player_head_mid_rags.dds</b>
(Pak3) <b>player_head_old.dds</b>
(Pak3) <b>player_head_rags.dds</b>

From the filenames, I'd be inclined to guess that <b>player_head_mid.dds</b> would probably be the main one, though.
