Ok either I have the worst luck, the game is bugged or I am doing something wrong.
Just started my second character as my first at the ripe old age of 32 was in very poor health. Went from Good to poor in just two years. Overall he had a good career but shorter then I would have liked.
The new character is just 22 years old and thinking I would like to play longer I took medicine as is speciality but I have already dropped to fine health. In neither case was I ever left on a island or imprisoned.
The one thing I have done is made each voyage 48 to 60 months so perhaps the length of the voyage has something to do with it. Any one who has any ideas I would sure appriciate it as I would like to get at least 20 years out of a character.
By the way I also had both items for health, and they if they were working correctly did not seem to help.
