The option should be in the Choose Character interface below the choose nation part of the interface. By default it says "Tutorial Deck", but you can change that with the arrows left and right of it. Is this option really not there???

There are two things with this mod that need to be confirmed: Can you buy large navy ships if you have a high enough rank and can you buy pirate ships at pirate shipyards if you join the pirates?

realistic health items: works, but surrealistic health items are still very common. not in chests, but they are on corpses. potions don't seem more expensive, but i could be wrong.

Perhaps the potions should be toned down some more then with the realistic health items mod on.

fixed trade missions: everything works. however, a notification of what is the quest cargo should be in the cargo menu, not in the trade menu.

improved santo domingo: works, but there is a rather large empty space where the market stalls used to stand.

That is intentional. Swindler did that to give Santo Domingo a more different look. The stalls could be re-enabled with a couple of codelines though. However, traders would then also need to be added again.
